The U.S. Embassy in Bogota is pleased to announce that representatives from the Regional Federal Benefits Unit will visit Bucaramanga, Medellin, Bogota and Pereira to offer services for beneficiaries or individuals with questions and applications on U.S. Social Security and other federal benefits.

Please include your name, SS number, telephone and service required. You must bring original documents as well as a photocopy of each document required.
WHAT TO BRING FOR SOCIAL SECURITY SERVICES (please bring legible copies of all documents to be submitted):
Applying for Social Security Benefits: Bring originals and copies of the following for all applicants: Birth Certificate, passport – if applying for auxiliary benefits or survivor’s benefits marriage and/or death certificates
Social Security Number Card Application: Bring a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS (https://www.ssa.gov/forms/ss-5fs.pdf).
Foreign Enforcement Questionnaire (FEQ): Colloquially known as “proof of life” forms, these were mailed to individuals that have a mailing address in Colombia registered with Social Security if the last two numbers of the Social Security under which you receive benefits are 00 through 49. If your benefits were suspended, please bring the filled out and signed SSA-7162 (https://uploads.mwp.mprod.getusinfo.com/uploads/sites/24/2023/01/SSA-7162.pdf) form and a valid ID if you receive benefits for yourself or a SSA-7161 (https://uploads.mwp.mprod.getusinfo.com/uploads/sites/24/2023/01/SSA-7161.pdf) form if you are a representative payee for someone else. We will be unable to accept forms for those whose last two digits are 50 through 99 as you will receive these in the mail around June/July 2023.
