U.S. Social Security – Federal Benefits Services Visit Cali-Medellin-Bogota- Barranquilla—July 31st to August 4th, 2023

The U.S. Embassy in Bogota is pleased to announce that representatives from the Regional Federal Benefits Unit will visit Cali, Medellin, Bogota and Barranquilla to offer services for beneficiaries or individuals with questions and applications on U.S. Social Security and other federal benefits.

Cali

Location: Centro Colombo Americano – Cali

Appointments: Please schedule appointment by sending an email to FBU.Santo.Domingo@ssa.gov on the subject line write: “APPOINTMENT SSA – COLOMBIA”. Please include your name, SS number, telephone and service required. You must bring original documents as well as a photocopy of each document required.

WHAT TO BRING FOR SOCIAL SECURITY SERVICES (please bring legible copies of all documents to be submitted):

Applying for Social Security Benefits: Bring originals and copies of the following for all applicants: Birth Certificate, passport – if applying for auxiliary benefits or survivor’s benefits marriage and/or death certificates

Social Security Number Card Application: Bring a copy of your valid U.S. passport, Certificate of Birth Abroad or original birth certificate and completed form SS-5FS.

Foreign Enforcement Questionnaire (FEQ): Colloquially known as “proof of life” forms, these were mailed to individuals that have a mailing address in Colombia registered with Social Security if the last two numbers of the Social Security under which you receive benefits are 00 through 49. If your benefits were suspended, please bring the filled out and signed SSA-7162 form and a valid ID if you receive benefits for yourself or a SSA-7161 form if you are a representative payee for someone else.
